TITLE V.

Every state has a Title V Program which is funded, in part, through the Federal Title V Block Grant that provides health-related services to children with special healthcare needs--sometimes abbreviated as "CSHCN"--and their families. Title V Programs also provide assistance in accessing other services that CSHCN and their families need, such as parent support and information networks. The financial eligibility criteria, residency criteria, and medical eligibility criteria for these Programs varies from state to state. These Programs are also known by different names in different states. The following is a list of contacts in each of the state Title V CSHCN Programs. The listing is based on the Directory of State Title V CSHCN Programs: Eligibility and Scope of Services. This report was developed by the Institute for Child Health Policy in Gainesville, FL, with financial support from the Maternal and Child Health Bureau, (US Department of Health and Human Services) and with technical assistance from the Association of Maternal and Child Health Programs. The information contained in this report is also available through the Internet. The address on the World Wide Web is http://www.ichp.edu.

IN EVERY ISSUE

EXCEPTIONAL PARENT'S "Search and Respond" column affords parents, caregivers, and professionals the opportunity to exchange information about their practical experiences in meeting the everyday challenges of life with a child or adolescent who has a disability. We try print as many Search and Respond letters as possible in each issue and on our Web site: http://www.eparent.com. You can write, fax, or e-mail your letters to: EXCEPTIONAL PARENT magazine, 555 Kinderkamack Road, Oradell, NJ, 07649; fax: (201) 634-6570; e-mail: epedit@aol.com; or visit http://www.eparent.com on the Web and click on "Search and Respond" which is located on the homepage. It is important to indicate whether the letter being sent is "Search" or a "Respond." When sending "Respond," please indicate the title, initials, month, and year of the "Search" letter you are answering. All "Responds" are also forwarded to the writers of the "Search" letters, and most are published. For spacing purposes, all letters may be edited. (As with all matters concerning medical care and advice, individuals are also expected to consult the appropriate professionals regarding care.)
