All of the 27 chapters are authored or co-authored by a group of 41 specialists, who teach and practice in the US, Mexico, India, Australia, and Canada. Beaty is with the U. of Tennessee's Campbell Clinic in Memphis; Kasser is with the Harvard Medical School and the Children's Hospital Boston.
