"As Americans live longer, acquire greater numbers of chronic conditions, and are prescribed more medications, the potential for hospitalizations due to adverse drug events rises. Increasingly, pharmacist-led interventions and outreach such as Novant's are instituted to reverse this trend. Novant Health's medication management approach to reducing readmissions in high-risk patients will be valuable for healthcare organizations wishing to curb the incidence of preventable rehospitalizations and preserve reimbursement levels."
