Do you know someone who you feel has gone above and beyond their duty of care to look after you or a loved one? If so, they could be our NEXT Health Hero!

The South Wales Echo has teamed up with Cardiff and Vale University Health Board and the award-winning Park Plaza hotel in Cardiff to pay tribute to the unsung NHS heroes in our area.

If you feel someone who has worked tirelessly to help a patient deserves recognition then this is your chance to tell us about them.

Just like last month's winner Helen Ludlow, pictured right, a nurse at University Hospital Llandough, they could win a fantastic pamper reward such as a spa treatment or a meal for two courtesy of Park Plaza hotel in Cardiff.

To nominate a Health Hero: Email in the online nomination form at to Catrin Pascoe, South Wales Echo, 6 Park Street, Cardiff, CF10 1XR.

All nominations must include: Your , contact details, the nominee's name and role, plus 200 words explaining exactly why you think they deserve to be our Health Hero.

If you have already nominated someone, they will be automatically entered into consideration for the next award.
