26 January 2018 - US-based home health and hospice provider Five Points Healthcare has merged with US-based home health provider Millenium Home Health Care, the companies said.
Millenium is a Medicare-certified home health provider based in Broomall, Pennsylvania with additional locations in West Pittston, Pennsylvania and Milford, Delaware.
The firm serves the greater Philadelphia market, northeast Pennsylvania and the state of Delaware.
Five Points said the combined company will operate under Five Points Healthcare, LLC.
Anthony Angelo, president and CEO of Millenium, will assume the role of president, Millenium Home Health and Rob Radics will remain the CEO of Five Points Healthcare.
Five Points Healthcare is based in Atlanta, Georgia and has home health operations in Alabama, North Carolina, Tennessee and Virginia along with hospice services in Tennessee.
Five Points was founded in 2011 by an experienced leadership team to build a leading regional home health and hospice provider by focusing on its five key stakeholders - patients, employees, physicians, providers and communities.
For 24 years Millenium Home Health Care has provided Medicare home health services to thousands of patients in Southeastern Pennsylvania, Northeastern Pennsylvania and the entire state of Delaware.
