To develop and implement a model for enhancing the opportunities and capacity of persons with serious mental health issues to acquire and sustain close relationships. $596,700 over three years.

Canadian National Institute for the Blind The Canadian National Institute for the Blind (CNIB), founded in 1918, is a volunteer agency and charitable organization dedicated to assisting the integration of the blind and visually-impaired of Canada into mainstream society, to improve their condition, and to prevent , Ontario Division. Toward equipment and accessibility costs in the five-year redevelopment of Lake Joseph Centre, in order to ensure that children and adults with disabilities can continue to enjoy the recreational facility. $75,000 over two months.

Hospice Association of Ontario The Hospice Association of Ontario (HAO) provides end-of-life care to terminal patients in the province of Ontario, Canada. It was founded in 1989 with eight members; as of April, 2005, it had over 150 member hospice facilities. , Toronto. Mood Disorders Association of Ontario, Toronto. To improve services to existing peer-support groups, establish new groups across the province, increase awareness within ethno-cultural communities, and reduce the stigma associated with mood disorders. $313,300 over three years.

Ontario Brain Injury Association, Toronto. To increase the support and information available to Ontarians living with the effects of Acquired Brain Injury A neurological condition, Acquired Brain Injury (ABI) is damage to the brain acquired after birth. It usually affects cognitive, physical, emotional, social or independent functioning and can result from traumatic brain injury (i.e. accidents, falls, assaults, etc. , and their families, by introducing community-level association memberships and strengthening the structure and capacity of the association. $360,000 over two years.

Ontario Co-operative Association. To produce a print and web-based map of Ontario cooperatives and credit unions, in order to create greater awareness of, and access to, the sector, and to encourage network building throughout the sector. $48,600 over six months.

Ontario March of Dimes Ontario March of Dimes (in French La Marche des Dix Sous de l'Ontario; officially Rehabilitation Foundation for the Disabled) is a Canadian charitable organization which provides programs and services to people of all ages with physical disabilities in Ontario. . To develop a Stroke Recovery Network, incorporating chapters of the Stroke Recovery Association of Ontario to achieve greater reach in providing peer support to stroke victims. $200,000 over three years.

Shelternet. To develop online services for abused women, expand web-based resources for children and families, and build the technological capacity of Ontario shelters to deliver online service. $210,500 over three years.

Strengthening Local Responses to Woman Abuse Through Provincial Collaboration Initiative c/o Violence Prevention Coordinating Council (Durham Region). To strengthen local woman abuse coordination efforts across Ontario by developing a provincial clearinghouse of information and supports, and by bringing local stakeholders together for a provincial gathering. $61,000 over 18 months.
