The interchange, jammed with commuters, was closed for two hours for investigation and cleanup.

No other serious injuries were reported, though five people were checked at hospitals, officers said.

Witnesses said Evans was traveling faster than the traffic flow. Subsequent investigation found that seven of 10 brakes on his tractor-trailer were out of adjustment, according to CHP officers. Evans was driving for San Jose-based Chano Trucking.

During Wednesday's investigation, Officers Don Pellegrino and Michelle Esposito recalled a similar accident on Nov. 17 in the same area involving the same truck, Sweeney said.

In that case, officials determined the truck's brakes were faulty.

Witnesses Wednesday said that as the truck Evans was driving rounded the curve, southbound on the transition road, it rear-ended a 1999 Chevrolet Cavalier driven by Cecilia Henkle, 48, of Valencia, Sweeney said.

The truck then swerved, rear-ended and sideswiped a 1999 Suburban driven by Danielle Doglash, 32, of Canyon Country, then sideswiped a 1990 Toyota Celica driven by Diana Goche, 45, of Saugus and a 1993 Ford Explorer driven by Robert Hagan, 37, of Agua Dulce. The Explorer was pinned between the truck and the guard rail over a 200-foot drop.

The tractor-trailer then scraped along the guard rail for 50 yards hitting three more vehicles: a 1998 Chevrolet van driven by Kenneth Johnson, 33, of Valencia, a 1989 Plymouth driven by Susan Fix, 50, of Agua Dulce and a big rig driven by Salvadore Montesdeoca, 41, of Sylmar.

James Hackbarth, 40, of Canyon Country came along at that point in a 1990 Mercedes-Benz and skidded into the second big rig.
