Otomycosis. (Otoscopic Clinic).


The otoscopic view shows a right ear with a 90% perforation of the pars tensa. The tympanum tympanum (tĭm`pənəm). The malleus extends from the superior annulus inferiorly, and the capitulum capitulum /ca·pit·u·lum/ (kah-pit´u-lum) pl. capi´tula   [L.] a small eminence on a bone, as on the distal end of the humerus, by which it articulates with another bone.  of the stapes and lenticular process of the incus are seen at the left upper quadrant left upper quadrant Physical exam The region of the body containing the stomach, spleen and tail of pancreas . A bluish-gray and a white mass of mycelia are seen on the anterior canal wall. Dried yellow pus is visible on the skin of the posterior canal wall.

Treatment involves profuse and frequent irrigation of the ear with 4% acetic acid (plain, white household vinegar) and subsequent tympanoplasty tympanoplasty /tym·pa·no·plas·ty/ (tim´pah-no-plas?te) surgical reconstruction of the tympanic membrane and establishment of ossicular continuity from the tympanic membrane to the oval window.  to correct the perforation. Treatment of otomycosis is topical. Systemic antifungal agents are rarely, if ever, required or valuable.
