This book was first published in 1996 by the Work Loss Data Institute, an independent organization free of political or commercial influence and entirely dedicated to providing comprehensive disability information. The Official Disability Guidelines integrates information from several U.S. Federal Government databases including the Centers for Disease Control and Prevention Centers for Disease Control and Prevention (CDC), agency of the U.S. Public Health Service since 1973, with headquarters in Atlanta; it was established in 1946 as the Communicable Disease Center.  National Health Interview survey conducted by the National Center for Health Statistics National Center for Health Statistics (NCHS) is part of the Centers for Disease Control and Prevention (CDC), which is part of the United States Department of Health and Human Services.
