C. Shieh, K. Topadze, J. Shea, V. Casagrande, R. Robinson, and K. Joos. Vanderbilt University School of Medicine, Nashville, Tenn. The free electron laser A free electron laser, or FEL, is a laser that shares the same optical properties as conventional lasers such as emitting a beam consisting of coherent electromagnetic radiation which can reach high power, but which uses some very different operating principles to form the beam. (EEL) at amide band II wavelength (7.7 [micro]m) has been shown to be more effective at cutaneous contraction than the [CO.sub.2] laser independent of collagen denaturation denaturation, term used to describe the loss of native, higher-order structure of protein molecules in solution. Most globular proteins exhibit complicated three-dimensional folding described as secondary, tertiary, and quarternary structures. . Therefore, this wavelength may be effective in closing trabeculectomy. Trabeculectomies with mitomycin C were done on 14 rabbits. A leaking hole was punctured in the bleb bleb (bleb) a large flaccid vesicle, usually at least 1 cm. in diameter. bleb n. A large flaccid vesicle. bleb a large flaccid vesicle, usually at least 0. and closed randomly with one of 7 techniques: suture, glue, Tisseel, Tisseel and glue, FEL FEL - Function Equation Language. The FEL was delivered through a hollow waveguide at 2 mL. The healing characteristics were followed for 1 month. Acute trabeculectomies, bleb holes, and closures were done on the contralateral eyes immediately before anesthesia. All trabeculectomies were examined histologically with serial sections. At 1 week, all suture treatments and tisseel combined with FEL, 7.7 [macro]m treatments, as well as one glue alone treatment showed complete healing. The other trabeculectomies had non-leaking holes with incomplete healing of the conjunctiva. Glue was applied to the holes, and all holes were covered with conjunctiva by 1 month. In a rabbit model of trabeculectomy bleb holes, faster healing occurred with suture or application of Tisseel combined with FEL treatment at 7.7 [macro]m. Tisseel treated with low FEL energy at 7.7 [macro]m appears useful in sealing leaking trabeculectomies. |
