Switching to eletriptan in migraineurs with unsatisfactory response to rizatriptan.Switching medications due to lack of efficacy or tolerability is a common therapeutic strategy for optimizing treatment. Eletriptan and rizatriptan, while in the same therapeutic class, differ in pharmacokinetic characteristics that may influence their clinical efficacy in migraine migraine (mī`grān), headache characterized by recurrent attacks of severe pain, usually on one side of the head. It may be preceded by flashes or spots before the eyes or a ringing in the ears, and accompanied by double vision, nausea, treatment. This study assessed treatment response to eletriptan 40 mg (E40) in migraineurs who were dissatisfied with rizatriptan. Patients (n = 123) meeting International Headache Society The International Headache Society (IHS) is a charity organisation founded in 1981 for people from all professions that are working to treat headache disorders. It has over 1,000 ordinary members (including national society members). criteria for migraine3, with 1-6 attacks/month and who reported "unsatisfactory treatment response" to rizatriptan within the year were included in an open-label study. Migraineurs took E40 to treat one attack, and were allowed a second dose if the headache recurred within 2-24 hours. The primary outcome was the percentage of patients exhibiting a response to treatment (either mild headache or pain-free) at 2 hours, prior to any rescue medication. Reasons for dissatisfaction with rizatriptan included inadequate pain relief (84%), high recurrence rate (69%) and lack of improvement in associated symptoms (60%). The headache response rate at 2h following E40 treatment was 64%; 30% were pain-free. Compared with baseline, more patients were free from migraine-associated symptoms at 2 hours post-dose--without nausea (78%), photophobia photophobia /pho·to·pho·bia/ (-fo´be-ah) abnormal visual intolerance to light.photopho´bic pho·to·pho·bi·a n. 1. (72%) or phonophobia (77%). Within 2 hours, 63% of patients with baseline functional impairment (moderate or severe) had a functional response (return to reduced or normal activities). Further, 42% of patients experienced a sustained headache response through 24 hours post-dose. Seventy-two percent of patients considered eletriptan as good-to-excellent treatment; 78% reported overall satisfaction with the degree of headache relief.
