Neuro-2. Early treatment of migraine: results of a prospective, double-blind, placebo-controlled trial using eletriptan.


Triptan trials in migraine migraine (mī`grān), headache characterized by recurrent attacks of severe pain, usually on one side of the head. It may be preceded by flashes or spots before the eyes or a ringing in the ears, and accompanied by double vision, nausea,  traditionally specified that attacks involve moderate-to-severe pain. However, retrospective analyses of patients taking triptans while pain was mild suggest that outcomes improve with earlier treatment. This study prospectively evaluated efficacy and tolerability of eletriptan early in the attack, while pain was still mild. Adult outpatients (n = 613) with diagnosed migraine with or without aura by International Headache Society The International Headache Society (IHS) is a charity organisation founded in 1981 for people from all professions that are working to treat headache disorders.

 to receive eletriptan 20 mg (E20), eletriptan 40 mg (E40) or placebo to treat one attack as soon as they were sure the headache was migraine, regardless of severity. Primary outcome was pain-free rate 2 hours post-dose. Two-hour pain-free rates were higher on E20 (35%) and E40 (47%) vs placebo (22%, P < 0.01). Most patients (63%) treated their headaches within 1 hour of onset; pain-free rates were similar between these patients (E20=37%, E40=46%, placebo=26%) and those who treated [greater than or equal to]1 hour (E20=33%, E40=47%, placebo=15%). Among the 25% of patients treating mild pain, pain-free rates were E20=36%, E40=68%, placebo=25% (P < 0.001, E40 vs placebo); when pain was moderate-to-severe, E20=35%, E40=39%, placebo=21% (P < 0.01), E40 and E80 vs placebo). Rates were highest when patients with mild pain took medication within 30 minutes of onset (E20=40%, E40=71%, placebo=23%). More patients taking E20 (70%) or E40 (75%) had functional response (returned to function normally or with reduced activities) compared with those taking placebo (45%, P < 0.001). Adverse events were generally mild-to-moderate in severity and occurred with similar frequency in all groups (E20=28%, E40=23%, placebo=32%). Treating migraine with eletriptan early in the attack provided better efficacy than reported in clinical trials with moderate-to-severe pain.
