Myringostapediopexy.


The otoscopic view is that of a left ear with a clearly abnormal appearance in an asymptomatic patient. Hearing is normal. This represents a myringostapediopexy. The short process of the malleus is the white structure at the top of the photograph. Extending vertically, inferiorly to the short process is the long process of the malleus. To the right of the umbo umbo /um·bo/ (um´bo) pl. umbo´nes   [L.]
1. a rounded elevation.

2. the slight projection at the center of the outer surface of the tympanic membrane.

 is a small, white structure representing the lenticular process of the incus incus /in·cus/ (ing´kus) [L.] the middle of the three ossicles of the ear, which, with the stapes and malleus, serves to conduct vibrations from the tympanic membrane to the inner ear. Called also anvil.  or capitulum capitulum /ca·pit·u·lum/ (kah-pit´u-lum) pl. capi´tula   [L.] a small eminence on a bone, as on the distal end of the humerus, by which it articulates with another bone.  of the stapes stapes /sta·pes/ (sta´pez) [L.] the innermost of the auditory ossicles; it articulates by its head with the incus and its base is inserted into the oval window

. The pars tensa is adherent to this structure, thereby conducting sound to the inner ear. The long process of the incus appears to be eroded. Superiorly, a small amount of epithelial debris can be seen.

Myringostapediopexy, when it occurs naturally, can be stable and function well. When hearing is normal, no treatment is necessary. Purposeful creation of myringostapediopexy surgically has been very difficult to accomplish. Other methods of ossicular reconstruction have been found to be more successful.
