Measured vs. calculated free T4.


Q Most automated chemistry analyzers now can directly determine free T4 (fT4). What are the advantages and disadvantages of replacing a calculated a free thyroxine index free thyroxine index FT4I, T7 assay, T12 assay Endocrinology A lab value for T3 uptake combined with total T4; FTI is a clinical parameter measured by RIA, used to evaluate thyroid function, calculated by T4 x %T  (FTI FTI Free thyroxine index, see there ) with a directly determined free T4. I find much of what I read confusing because it seems authors, sometimes, will not clearly state whether they are talking about FTI or directly determined free T4.

A The FTI has been replaced in most cases by a free T4 measurement because it tends to be less susceptible to abnormal results at the extremes of measurement of the total T4 or the T3 resin binding (the two assays that make up the calculated FTI).

Most thyroidologists no longer use the FTI except in unusual cases (non-thyroidal illness, for one example, because of circulating endogenous endogenous /en·dog·e·nous/ (en-doj´e-nus) produced within or caused by factors within the organism.
