"Physicians have grown very comfortable scribbling out illegible, incomplete and error prone orders," says Brian Jacobs, M.D., director of technology and patient safety at Cincinnati Children's Hospital Medical Center Cincinnati Children's Hospital Medical Center is a hospital in Cincinnati, Ohio.
