Environment and health: capacity building for the future/ Environnement et sante: augmenter le potentiel pour le futur.

In October 2002, the World Health Organization (WHO) sponsored a workshop in Geneva, Switzerland, to address the problems faced by medical journals in the developing world regarding their efforts to provide critical, timely health information to local health practitioners and research scientists. Of major concern is the unavailability of the results of medical research from developing nations, which is published in international journals, to those who are living and working in developing nations. If scientific knowledge is to be used to improve public health and the environment, then it must be accessible to the local health professionals who need it most. A specific outcome of this workshop was the creation of the Forum for African Medical Editors (FAME) with 12 inaugural African medical editors, both anglophone and francophone. While the WHO is a major sponsor of FAME, other participating organizations, institutions, journals, and associations offer various forms of assistance.

In September 2003, the U.S. National Institutes of Health (NIH)--specifically, the National Library of Medicine (NLM), the Fogarty International Center, and the National Institute of Environmental Health Sciences (NIEHS)--sponsored a meeting in London at the offices of the British Medical Journal (BMJ). The primary objective was to discuss the partnership of four sub-Saharan African medical journals with five Northern Hemisphere medical journals as a mechanism to enhance the quality and credibility of the African journals and thereby attract high-level research.

Identified steps needed to enhance the quality of the African journals included providing training for editors in improving sustainability and publishing regularity, improving the peer-review process by identifying experienced reviewers willing to serve and mentoring new reviewers, and offering local researchers guidance in preparing research papers for publication. Ideas for improving the credibility of the African journals included having respected scientists from multiple countries serve on the journals' editorial boards, earning inclusion in major indexing databases such as the NLM's MEDLINE, and exploring ways to share journal content, for example, by copublishing peer-reviewed research articles of high importance to the people in the area served by regional journals. This latter approach would have the added benefit for researchers and health practitioners in developed countries of making important regional research results more available in the international literature.

In May 2004, a contract was awarded to the Council of Science Editors to manage the funds for a pilot project intended to build the capacity of the four sub-Saharan African journals as per the thoughts generated at the London meeting. The African journals were selected because all of their editors are founding members of FAME and thus are committed to enhancing the capacities of their journals as well as other sub-Saharan medical journals. In addition, the African journals are published in countries that have active NIH-sponsored research and are also part of the communication network developed by the NLM for the Multilateral Initiative on Malaria. The Northern Hemisphere journals were selected on the basis of their missions and commitment to advancing public health and the environment in developing regions of the world. The following four journal partnerships have been established: a) African Health Sciences with BMJ; b) Ghana Medical Journal with The Lancer; c) Malawi Medical Journal with the Journal of the American Medical Association; and d) Mali Medical with Environmental Health Perspectives (EHP) and the American Journal of Public Health. The last partnership--ours-is the only one involving two Northern Hemisphere journals and the only one involving a francophone journal.

In September 2004, the three of us met in Research Triangle Park, North Carolina, to begin working toward the successful completion of out contract tasks, which were as follows:

1. To identify the equipment and facility needs of the Mali Medical and then provide computer hardware and software to the publishing offices, along with initial training for editorial office personnel

2. To identify the editorial needs of the Mali Medical through mutual site visits by the partnering editors-in-chief to observe editorial and publishing practices

3. To provide author/reviewer training via workshops, emphasizing international standards for writing and systematic approaches for reviewers, open to all FAME members at scheduled scientific/medical meetings in Africa

4. To provide training and support for a managing editor/business manager in establishing business plans for effective, sustainable publishing operations through technical consultation and through a workshop in Africa open to all FAME members

5. To develop and maintain a website that would permit online publication of the Mali Medical

6. To establish internships for representatives of the Mali Medical at the editorial offices of EHP and the American Journal of Public Health

7. To commission four systematic reviews on topics relevant to sub-Saharan Africa to be published in partnering African journals in both English and French.

Over the next several years, we plan to evaluate the success of our capacity-building initiative using the following indicators: progress toward indexing the Mali Medical in MEDLINE, numbers of articles submitted and published, numbers and effectiveness of local peer reviewers, and timeliness of publication. If our journals and sponsoring organizations are to fulfill our common missions of working to improve the public's health and achieving equity in health status for all, then our nascent partnership is a viable means toward this end. Our collective hope is that all three journals will better realize their potential to serve as vehicles for progressive change through increased understanding, collaboration, insight, and connections between the environment and health in the developed and developing world. Look for regular updates from us published simultaneously in all three journals in both English and French. We aim to hold one another accountable in fulfilling our assigned tasks and enlisting other partners in our rewarding struggle to find creative and practical solutions that eliminate past and present health inequalities and protect the environment for future generations.

En octobre 2002, l'Organisation Mondiale de la Sante (OMS) a parraine un atelier a Geneve, en Suisse, s'attaquant aux problemes rencontres par les revues medicales dans les pays en developpement dans leurs efforts pour fournir une information medicale importante en temps voulu aux chercheurs et praticiens locaux. Le probleme le plus inquietant est le manque d'acces pour ceux qui vivent et travaillent dans les pays en developpement aux resultats de la recherche medicale provenant des memes pays en developpement et publies dans des revues internationales. Si le but de la science est l'amelioration de la sante publique et de l'environnement, alors elle doit etre accessible aux professionnels locaux de la sante qui en ont le plus besoin. A l'issue de cet atelier fut cree le FAME (Forum des Redacteurs en Chef Medicaux Africains), qui comptait a son inauguration 12 redacteurs en chef africains, anglophones et flancophones. Bien que l'OMS soit le principal sponsor du FAME, d'autres organisations, institutions, revues et associations participantes apportent leur assistance sous differentes formes.

En septembre 2003, les Instituts nationaux americains de la sante (NIH)--a savoir, la Bibliotheque nationale de medicine (NLM), le Centre international Fogarty et l'Institut national des sciences de l'hygiene du milieu (NIEHS)--ont organ ise un meeting a Londres, dans les bureaux du British Medical Journal L'objectif premier etait de discuter d'une collaboration entre quatre revues medicales d'Afrique sub-saharienne et cinq revues medicales de l'hemisphere Nord comme moyen d'ameliorer la qualite et la credibilite des revues africaines, et d'attirer ainsi une recherche de haut niveau. Les etapes definies comme necessaires a une plus grande qualite des revues africaines comprenaient: former les redacteurs en chef a ameliorer la rentabilite et a regulariser leur publication; ameliorer le procede de lecture en trouvant des lecteurs experimentes desireux de former et de suivre de nouveaux lecteurs; proposer aux chercheurs locaux des conseils pour la preparation a la publication de dossiers de recherche. Les idees pour l'amelioration de la credibilite des revues africaines incluaient la participation aux editoriaux de scientifiques reconnus originaires de differents pays; etre d'avantage inclus dans les grandes bases de donnees telles que Medline de la NLM, et chercher le moyen de partager le contenu des revues, c'est a dire co-publier, apres relecture, des articles de recherche de haute importance pour les populations des regions concernees par les revues. Cette derniere approche beneficierait egalement aux chercheurs et praticiens des pays developpes, en rendant les resultats importants de la recherche regionale plus accessibles dans la litterature etrangere.

En mai 2004, un contrat fut concede au CSE (Council of Science Editors) pour la gerance de fonds d'un projet pilote visant a augmenter le potentiel de quatre revues d'Afrique sub-saharienne selon les idees emises lors du meeting de Londres. Les revues africaines selectionnees furent celles dont les redacteurs en chef etaient membres fondateurs du FAME, et qui s'etaient donc engages a ameliorer la qualite de leurs revues ainsi que d'autres revues medicales sub-sahariennes. De plus, ces revues africaines sont publiees dans des pays qui ont une recherche active sponsorisee par des NIH, et qui font egalement partie du reseau de communication mis en place par la NLM pour le Multilateral Initiative on Malaria. Les revues de l'hemisphere Nord furent selectionnees en fonction de leurs missions et engagement dans l'avancee de la sante publique et de l'environnement dans les regions en developpement. Les quatre collaborations etablies entre revues sont les suivantes: a) African Health Sciences et le British Medical Journal; b) Ghana Medical Journal et The Lancet; c) Malawi Medical Journal et le Journal of the American Medical Association; d) Mali Medical, Environmental Health Perspectives et 1' American Journal of Public Health. La derniere collaboration--la notre-est la seule qui inclut 2 revues de l'hemisphere Nord, et la seule incluant une revue francophone.

En septembre 2004, nous nous sommes reunis au Research Triangle Park, en Caroline du nord, aux Etats-Unis pour commencer a travailler au succes de l'accomplissement de nos taches contractuelles, qui sont:

1. Identifier les besoins en equipement du Mali Medical puis fournir du materiel informatique et des logiciels aux redactions, ainsi qu'une formation de base au personnel de la redaction 

2. Identifier les besoins editoriaux du Mali Medical au moyen des visites mutuelles de la part des redacteurs en chef associes, afin d'observer les methodes de redaction et d'edition

3. Offrir une formation d'auteur/lecteur au moyen d'ateliers, en mettant l'accent sur les normes internationales d'ecriture et les approches systematiques de lecture, ouverte a tous les membres du FAME lors de sommets medico-scientifiques en Afrique

4. Offrir une formation et un support au redacteur en chef et au directeur du service commercial en etablissant des plans pour des operations d'edition efficaces et viables grace a une consultation technique et un atelier en Afrique ouvert a tous les membres du FAME

5. Developper et entretenir un site Internet qui permettrait la publication en ligne du Mali Medical

6. Organiser des stages pour les representants du Mali Medical au sein des redactions de l'Environmental Health Perspectives et de l'American Journal of Public Health

7. Mettre au point quatre compte-rendus systematiques sur des sujets importants en Afrique sub-saharienne qui seront publies dans les revues africaines partenaires a la fois en anglais et en francais.

Au cours des annees a venir, nous evaluerons le succes de notre initiative d'amelioration de potentiel au moyen des indicateurs suivants : augmentation des references au Mali Medical dans Medline, nombre d'articles soumis et publies, nombre et efficacite des lecteurs locaux, et ponctualite de publication. Si nos revues et organisations partenaires accomplissent nos missions communes que sont travailler a l'amelioration de la sante publique et parvenir a l'egalite dans la sante pour tous, alors notre collaboration naissante est un moyen efficaoe pour atteindre ces buts. Notre espoir commun est que les trois revues exploitent mieux leur potentiel d'acteurs d'un changement progressif, et ce a travers une plus grande comprehension, la collaboration, la perspicacite et les relations entre l'environnement et la sante dans les pays developpes et en developpement. Nous mettrons a jour regnlierement et simultanement nos trois revues, en anglais et en francais. Nous souhaitons etre chacun tenu responsable pour l'accomplissement des taches qui nous sont assignees et l'engagement d'autres partenaires dans ce combat digne d'etre mene: trouver des solutions pratiques et innovantes pour eliminer les inegalites medicales passees et presentes et proteger l'environnement pour les generations futures.
