Perdikis also was found negligent in a spinal surgery on another patient in 1997 for failing to remove a sufficient amount of disk material.

The charges were in a complaint filed on behalf of the Medical Board by the state Attorney General's Office.

``(Perdikis') acts of dishonesty are inconsistent with his established reputation for honesty and integrity. It therefore appears that those acts may have been an anomaly in his professional practice,'' wrote Waxman in his ruling. ``(Perdikis) has resolved to change his operative report operative report A document produced by a surgeon or other physician(s) who have participated in a surgical intervention, which contains a detailed account of the findings, the procedure used, the specimens removed, the preoperative and postoperative diagnoses, and  writing to ensure that he provides more accurate information than he did in (the tumor patient's) case.''

Waxman issued his ruling last month after a nine-day hearing in June, and it was adopted by the state Medical Board last Wednesday. The decision takes effect Oct. 15.

Generally probation is ordered for a specified period of time during which the physician must comply with terms and conditions and report to a Medical Board probation monitor. Practice is permitted unless otherwise specified.
