Children, accompanied by a parent or legal guardian, will be inoculated against polio (DTaP), measles, mumps and rubella rubella or German measles, acute infectious disease of children and young adults. A gram-negative, rod-shaped bacterium of the genus Haemophilus that is found in the human respiratory tract and causes acute respiratory infections, such as pneumonia, and other diseases,
 (HiB), tetanus diphtheria diphtheria (dĭfthēr`ēə), acute contagious disease caused by Corynebacterium diphtheriae (Klebs-Loffler bacillus) bacteria that have been infected by a bacteriophage. It begins as a soreness of the throat with fever.  (TD), hepatitis A and hepatitis B. A tuberculosis skin test, required to attend school, will also be available. An immunization record is required for each child, otherwise a record will be provided for each youngster vaccinated.
