Courage Center Accepting Nominations for Annual Jay and Rose Phillips Awards; Awards Honor People with Disabilities Who Have Achieved Outstanding Vocational Success.MINNEAPOLIS -- Courage Center, a rehabilitation rehabilitation: see physical therapy. and resource center for people with disabilities, is accepting nominations for its annual Jay and Rose Phillips Awards. The awards are presented each year by Courage Center to people with disabilities who have achieved outstanding success in their vocation. Winners receive a special plaque plaque (plak) 1. any patch or flat area. 2. a superficial, solid, elevated skin lesion. attachment plaques , a substantial cash award and are recognized at Courage Center's annual gala Celebration of Courage. This year's Celebration is set for Saturday, August 27 at Medtronic Medtronic Cardiology A major manufacturer of pacemakers, defibrillators, and other cardiac devices World Headquarters, 701 Medtronic Parkway, Fridley, Minn. Nominees for the award must have a physical disability and be vocationally and financially independent (Medical Assistance excluded) for a minimum of three years. Self-employed individuals are also eligible for the award. Employers are encouraged to nominate nom·i·nate tr.v. nom·i·nat·ed, nom·i·nat·ing, nom·i·nates 1. To propose by name as a candidate, especially for election. 2. To designate or appoint to an office, responsibility, or honor. their employees who meet these criteria.
