To notify the department of your intent to self-certify, the form can be submitted on-line, faxed to 212-566-4594 or 212-566-4752, mailed or dropped-off at 280 Broadway. To submit inspection results the form must be mailed or dropped off. When mailing or dropping off your form, please send the original plus 2 copies and required documentation along with a stamped self-addressed envelope. Beginning Jan.20 all inspection schedules, results, objections and status will be available on online. |
