For these demonstration projects, the State was seeking innovative approaches to disease and care management in an effort to promote and support the physician/patient relationship for quality health care delivery, improved understanding of treatment options and increased patient involvement in managing their own care. These principles are at the foundation of CareGuide/Patient Infosystems' programs, which emphasize high-touch, personal interactions with participants and improved communication and collaboration among the care team. The State will evaluate each regional program in order to better understand the efficacy of the various care models.
