Cappella Inc. has developed both a drug delivery device and technique for approaching and treating patients with ostial-bifurcation lesions. The Sideguard ostium protection device is a self-expanding coronary drug-eluting stent technology that is deployed into the sidebranch first.

The Sideguard's trumpet-shaped design helps the stent conform to the ostium, allowing for complete stent-to-wall apposition and continuous drug delivery. This alleviates many of the problems associated with deploying multiple stents simultaneously, yet leaves the parent and side-branch free for future stenting.

"What makes Sideguard different than conventional DES or dedicated bifurcation stent technologies is Sideguard's concept and design," states Neev. Sideguard will be the first commercialized dedicated side-branch stent with a built-in circumferential ostium support feature.

Professor Donald Baim, who recently presented his experience in the animal lab with Cappella, stated that "IVUS, radiography and histology show excellent expansion in both the side-branch and the main vessel, full lumenal patency, and complete wall apposition through the ostial transition segment. This should translate into uniform drug-delivery and low clinical restenosis rates."

"Our stent is the first nitinol balloon-released, sheathed coronary stent. Using nitinol (a memory metal) allows us to pre-shape the stent before crimping it to the delivery system. Then, a protective sheath is applied to keep it in place until deployment. Once it's released, it expands into place and its pre-shaped trumpet design allows the stent to hug to the vessel wall and ostium, providing complete apposition and continuous drug delivery," points out Guy Neev.

According to Professor Campbell Rogers, a Sideguard principal investigator, the Sideguard's ease of delivery, favorable acute angiographic results (as reported in animal studies), and its seamless apposition (as demonstrated by both IVUS and histology) make Sideguard a potential alternative to current bifurcation stent practices and make it an appealing platform for drug delivery.
