Kowalski's third child, Johnathan, 1, was treated and released from Simi Valley Hospital.

The accident happened Wednesday afternoon at Cochran Street and Justin Avenue, police said. Kowalski was driving east on Cochran and was making a left turn onto Justin.

A westbound Pacific Beverage Co. tractor-trailer driven by William Karrasch, 34, at 25 to 35 mph slammed into the van on the passenger side, said Sgt. Jeff Malgren.

``The different sizes of the vehicles usually indicate there is going to be severe damage, and along with that is usually a loss of life,'' he said. ``I just think I'm surprised we didn't have a more tragic result at the scene.''

Malgren urged motorists to pay special attention to oncoming traffic when making left turns, and not to risk their lives to save only a few seconds.

``Don't be afraid to wait if you're not sure of the speed of oncoming cars,'' he said. ``It's better to wait for the next signal phase than to try to get through on a green or yellow light.''
