$121,905, to provide cultural outreach,
education and health care services, with a
focus on diabetes, to San Mateo County's Latino
population. Using the "promotores" model,
community members will be trained to assist
people from their neighborhoods in preventing,
identifying and dealing with their diabetes.
o Corporation for Supportive Housing, Oakland,
$200,000, for comprehensive primary health
care, mental health and substance abuse
treatment for homeless and at-risk adults with
disabilities in a supervised housing setting.
o Open Door Community Health Center, Humboldt
County, $78,911, for a pilot program to
establish preventive, primary and dental care
system in three rural communities. Community
coordinators will establish new clinics to meet
the needs of each site.
o John F. Kennedy University South Bay Community
Counseling Center, Orinda, received a second
year grant of $97,245, for comprehensive
school-based mental health services for
children and families in 23 Santa Clara County
elementary schools.
o AIDS Healthcare Foundation, Los Angeles,
$150,000, to expand prevention services at five
outpatient service centers and to target the
East Los Angeles community, focusing on Latino
injection-drug users.
o Gay & Lesbian Adolescent Social Services, Inc.,
West Hollywood, $153,402, for Mobile Health
Outreach Project to provide primary health
care, HIV/AIDS prevention and screening for
youth living on the streets of Hollywood and
West Hollywood.
o Greater Los Angeles Council on Deafness, Inc.,
Los Angeles, $107,496, to improve health care
delivery to deaf consumers by developing a
collaborative relationship between the health
care and deaf communities.
o Vietnamese Community of Orange County, Inc.,
Santa Ana, $100,000, to develop a community
clinic utilizing existing health care resources
to maximize health services for Vietnamese
women and children.
o Human Options, Inc., South Orange County,
$75,000, to train medical personnel to
recognize and intervene with abused women and
their children and provide counseling for
children exposed to the trauma of domestic
violence.
o East Oakland Over Sixty Health Center, Oakland
and Berkeley, $150,000, to expand primary and
mental health services to low-income seniors in
the East Bay and San Francisco Bay areas.
o Asian Health Services, Oakland, $140,204, to
expand health services for low-income Asian
immigrants and refugees.
o Community Health Clinic Ole, Napa, $59,038, to
increase primary care for infants and children
among the Spanish-speaking and medically
underserved population of Napa Valley.
o Blind Babies Foundation, San Francisco,
$50,000, to provide support, education and a
variety of developmental activities to visually
impaired children aged 0 to 3 and their
families.
