TWO Warwickshire mums were killed by their mentally ill partners after a string of failings by health workers, a damning report has revealed.

Fiona Stephenson was stabbed to death by her husband Owen in a frenzied attack at their home in Rugby in January, 2005.

And just four weeks later Colette Lynch, also of Rugby, was knifed in the street by her former boyfriend Percy Wright. A report to health trust NHS Warwickshire yesterday reveals mental health workers missed several chances to prevent the killings and flags up a list of urgent changes required to stop similar tragedies occurring.
