Award nomination: dos & don'ts.


DO ... Submit all awards to the US Field Artillery Association no less than 30 days before presentation date to avoid late fees.

DO ... Use the proper forms and fill out completely to avoid any delays in processing.

DO ... Double check all spelling of nominees' names. (Replacement certificates are $5.00 ea.)

DO ... Include full payment for all nominations. Orders received without full payment will not be processed.

DO ... Include only one form of payment per bulk order.

DO ... Submit bulk orders (5 or more) on a Multiple Nomination Form. Please submit Saint Barbaras & Molly Pitchers on the same memorandum for multiple nominations. (See example on Form 200-M on Page 52)

DO ... Include complete and correct shipping address. We are not responsible for delays in shipping or customs due to an incomplete or incorrect address.

DO ... Include a "Letter of Recommendation" for all Ancient Order of St. Barbara nominations. All recommendations will be approved or disapproved by the Chief of Field Artillery.

DO ... Include a "Letter of Recommendation" for all Honorable Order of St. Barbara and/or Artillery Order of Molly Pitcher nominations that are not approved by a Field Artillery O-6 or above in command. These recommendations will be approved or disapproved by the Chief of Field Artillery.

DON'T ... Submit multiple memorandums for bulk orders; have one memorandum for all units and nominees. Orders will not be processed as a bulk order if more than one memorandum is submitted.

DON'T ... Have more than one point of contact. Numerous persons calling can result in miscommunication and cause delays.

DON'T ... Have more than one form of payment. Only one form of payment will be accepted per order.

DON'T ... Have multiple shipping addresses. We will not process orders with multiple shipping addresses as one bulk order. These orders will be processed and shipped separately. (This may result in extra fees and/or loss of "Bulk Order" discount.)

Orders are shipped 2 weeks prior to presentation date unless otherwise specified. You may request an earlier ship date if time permits, but be mindful of the 30-day rule. Late orders will not be rushed and may arrive within a few days prior to or after the presentation date.
