NOMINATION OF GUARDIAN FOR A MINOR

I, --, the legal parent of the minor child -- who was born on --, am hereby declaring my wishes as to those individuals to be appointed the legal guardian of the person and property of my (daughter/son) in the event I am unable, physically or mentally, to care for my child.

I.

This nomination is based on the fact that a loving and parental relationship exists between -- and my (daughter/son), -- --. Furthermore, my (daughter/son) has lived with this adult and looks to her for guidance, support, and affection.
