Allergy to folic acid.


In a case report, a woman had three episodes of allergic reactions, including anaphylaxis anaphylaxis (ăn'əfəlăk`sĭs), hypersensitive state that may develop after introduction of a foreign protein or other antigen into the body tissues. , after ingesting synthetic folic acid. Intradermal intradermal /in·tra·der·mal/ (-der´mal)
1. within the dermis.

2. intracutaneous.

 testing with folic acid was positive. A graded, blinded challenge with a folic acid solution led to widespread urticaria urticaria /ur·ti·ca·ria/ (ur?ti-kar´e-ah) hives; a vascular reaction of the upper dermis marked by transient appearance of slightly elevated patches (wheals) which are redder or paler than the surrounding skin and often attended by  at a dose of 160 mcg. The patient appeared to tolerate dietary folates (pteroylpolyglutamates).

Comment: Although allergy to synthetic folic acid is rare, it is likely to become a more common clinical entity now that grains are being fortified with this vitamin. Folic acid allergy should be considered in the differential diagnosis of idiopathic anaphylaxis and in patients with suspected grain allergy.
