Affiliated Community Medical Centers, Willmar, MN, Sept. 17 (Minneapolis). Level 1 non-compliance included missing QC records and an out-of-limits film processor. Level 2 deviations included lack of documentation of corrective action on a processor, lack of continuing medical education (CME) requirements and lack of procedures for complaint handling. The facility was inspected by a representative of the State of Minnesota on Aug. 31.
