Administrative fellowships in emergency medicine.


Arecent study of physician executives found that advanced training beyond normal work experience is needed for physician executives.(*) Currently, physician executives receive didactic managerial experience through continuing medical education continuing medical education See CME.  courses or by management degree programs. The Emergency Physicians Medical Group (EPMG EPMG Emergency Physicians Medical Group (staffing and management; Roseville, CA) ) first began its fellowship in 1990. EPMG is an emergency medicine management group based in San Francisco San Francisco (săn frănsĭs`kō), city (1990 pop. 723,959), coextensive with San Francisco co., W Calif., on the tip of a peninsula between the Pacific Ocean and San Francisco Bay, which are connected by the strait known as the Golden , with 14 community and academic hospital contracts located in California, Hawaii, and Nevada. The fellow performs approximately 80 clinical hours per month, performing administrative duties for the remainder of the time. The fellowship length is approximately one year. The curriculum includes:

* Prehospital care

* Billing and reimbursement

* Quality management programs

* Personnel development

* Leadership skills development

* Program development

* Building/maintaining effective interdepartmental in·ter·de·part·men·tal

 quality management, risk management, billing, reimbursement, and marketing. The EPMG fellowship also offers the opportunity to participate in prospective contract negotiations throughout the year.

The EMSCO Management Services administrative fellowship is a one-year program following completion of an emergency medicine residency. EMSCO Management Service is an emergency medicine management group based in Chicago. Its fellowship program was founded in 1993. The fellow learns the essentials of administration through an affiliation with a group of emergency medicine physicians based at several hospitals in the Chicago area. The fellow also performs approximately 1,360 clinical hours during the fellowship. At completion of the program, the fellow will have experienced functioning as "assistant director" at three different hospitals. The curriculum includes:

* Negotiations/conflicts

* Reimbursement issues

* Managed care issues

* Emergency department management

* Hospital administration/interactions

* Prehospital care

* Physician recruitment and retention programs

* Medical defense systems (malpractice claim liability assessment)

* Quality improvement programs

* Risk management

* Professional liability insurance products/coverage

* Educational/teaching responsibilities

* Occupational medicine/health systems

* Leadership style and communication skills

* Emergency medicine practice/cost containment

The fellow has specific assignments in each curriculum category prior to completion of the fellowship. Specific reading lists and attendance at specific CME courses are required. Compensation of approximately $104,000 for clinical and administrative duties is provided, along with malpractice, health, life, dental, and disability insurance. Office space and research materials are also provided during the fellowship.

The Orlando Regional Medical Center The Orlando Regional Medical Center (ORMC) is the only level I trauma center in the Central Florida area and, therefore, provides intensive care for this area. ORMC is part of a group of hospitals known as Orlando Regional Healthcare, and itself is home to three other  (ORMC ORMC Orlando Regional Medical Center (Orlando, FL)

) fellowship also began in 1993. The fellowship is one or two years in length, depending on individual needs. The fellowship is based at Orlando Regional Healthcare Orlando Regional Healthcare is a system of non-profit hospitals serving the Greater Orlando area. There are nine hospitals in the system in six locations.

The headquarters for the system are located on the campus of Orlando Regional Medical Center south of Downtown Orlando,
 System, a not-for-profit organization with five hospitals and emergency department visits ranging from 10,000 to 65,000 per year. The fellowship is cosponsored by Emcare, Inc., an emergency medicine management group based in Dallas, Tex. The fellow is primarily based at ORMC, a teaching hospital with seven residency programs, including emergency medicine. The fellow participates as an attending in the emergency medicine residency program, supervising and instructing residents for no more than 90 hours per month. The remainder of the time is spent performing administrative duties under the direction of the medical director and the director of medical education. The curriculum includes:

* Risk management

* Total quality management

* Patient complaints

* Billing/reimbursements

* Prehospital care

* Medical/legal issues

* Credentialing

* Scheduling

* Research in cost containment/practice guidelines

* Development of diagnostic guidelines for physician order entry

* Lectures to emergency medicine residents and attendings

A specific reading list is given at the beginning of the program. The fellow has have the opportunity to participate in administration from an academic perspective in a community-based hospital. The fellow will also have extensive exposure to issues regarding managed care, including prospective contract negotiations. Compensation includes approximately $110,000/year for administrative and clinical duties, including malpractice, health, life, dental, and disability insurance. Other financial support is given for education conferences and research paper presentation.

Conclusion

Today, only these three administrative fellowships exist for physicians, all in emergency medicine. Others are being implemented (see accompanying box). Administrative fellowships should be considered in all specialties of medicine to prepare physicians for expanding roles as physician executives and managers. It simply requires a belief by physicians, health care organizations, and hospitals, that physicians need to stay involved in the management of hospitals and other health care organizations during these times of health care reform.
