Those locations are: < CMHA Riverside Park Community Center < 17800 Parkmount Avenue, Cleveland 44135 < < CMHA Riverview Community Center < 1795 West 25th Street, Cleveland 44113 < (Just north of Riverview Tower) < < CMHA Severance Tower Apartments < 25 Severance Circle, Cleveland Heights 44118 The lottery, which opened Monday, September 25th, provides the means for CMHA to establish a new waiting list for the Housing Choice Voucher Program for use after the current list is exhausted. Individuals who have not already obtained a form should pick-up and complete only one entry.
