ActiveHealth Management Receives NCQA Accreditation for Disease Management.


Informed Care Management Program Awarded 3-Year Accreditation for 20 Chronic Conditions

NEW YORK -- ActiveHealth Management (ActiveHealth([R])), a clinically-based, technologically-driven health management services company, announced today that its disease management program, Informed Care Management (ICM), received full Patient and Practitioner Oriented Accreditation from the National Committee for Quality Assurance National Committee for Quality Assurance Medical practice A private, not-for-profit organization which has become the leading accreditor of managed care plans; in site visits, NCQA reviewers evaluate a managed care plan in terms of quality management, physicians'  (NCQA NCQA National Committee on Quality Assurance, see there ). The NCQA is the gold-standard source for measuring the quality of domestic healthcare. ActiveHealth's exemplary scores in the NCQA review process earned the company a three-year accreditation for twenty disease management programs. This represents more NCQA-accredited conditions than any other disease management organization.

ICM is powered by ActiveHealth's CareEngine([R]) System, a patented clinical decision support technology that continuously scans all available data, including health plan-provided medical, pharmacy and lab claims; member self-reported data; and provider feedback data, and compares this member-specific information to the latest findings in evidence-based literature. When an opportunity for better care is discovered, a clinical alert is sent to the physician and member to help avoid medical errors and prevent unnecessary hospitalizations.

ActiveHealth also uses the CareEngine System to identify members that can benefit the most from targeted disease management, and gives its Nurse Care Managers unique access to member-specific information to guide their communications. ICM nurses perform health assessments on identified members, and prepare and communicate a personalized, actionable plan of care that addresses all of a member's identified health conditions, including discussion of any clinical alerts identified by the CareEngine. The ICM health assessments are highly structured and comprehensive, and are delivered via a proprietary system, called ActiveAdvice([R]), that ensures reproducibility and standardization. Information obtained from the member in ICM is fed back into the CareEngine, and is integrated with the other data streams to generate more clinical alerts.
