Access Health to Provide Care Management Services to Santa Clara Family Health Plan.
Under the agreement, members of Santa Clara Family Health Plan will have around-the-clock telephone access to symptom assessment, advice on appropriate care options and health information provided by specially trained registered nurses.
"We are very pleased to work in partnership with Santa Clara Family Health Plan to provide their health plan members with immediate access to health information and counseling through our 24-hour nurse advice line," Access Health Chief Executive Officer Thomas E. Gardner said.
"Our services complement the other health promotion efforts Santa Clara Family Health Plan has in place and will help them meet their objectives in the areas of health education, cost management and member satisfaction."
Santa Clara Family Health Plan (SCFHP), a San Jose-based HMO, provides health care services to Medi-Cal beneficiaries in Santa Clara County. Medi-Cal is California's version of the federal Medicaid program. An estimated 60,000 members will be enrolled in Access Health's care management service, slated to begin in mid-March.
