The report suggests all the decisions taken were right, based on the evidence at the time. A series of recommendations have been made which will no doubt be followed. But on first reading, this report will do nothing to assure the public appropriate assessments and procedures are in place to prevent such a tragedy occurring again. The fact is an innocent woman died at the hands of a man with a history of mental health problems just hours after he was released from hospital. There is nothing in the report which gives a satisfactory answer to the fundamental question. Why?
