'Safety flaws' over Top Gear crash.AN INVESTIGATION into the high-speed crash which nearly claimed the life of Solihull-born presenter Richard Hammond has identified "failings in the BBC's safety management systems". But it concluded no-one should be prosecuted over the incident. The crash happened in September last year while Hammond was driving a Vampire, a jet-powered drag-racer capable of nearly 300mph. The Top Gear presenter survived the crash at Elvington airfield, near York.
