"Never events" back in the news.

Two years ago, Minneapolis-based HealthPartners Inc. made headlines by requiring providers who serve their members to report the commission of "never event" medical errors to the state health department and to not bill members for these errors. Now, never events are back in the headlines. Today, 11 states use this information as the foundation of their public accountability mandate for reporting to state health departments or reporting authorities.
